Give your three preferences for nights of play and ratios. Keep in
mind that Monday to Wednesday nights are full and spots will only be available
if teams do not return from last year. Spots will be awarded on
a first come first served model based on the receipt date of this form. Please
note that first choices take precedence over second, etc. This means that if
you pick a Monday spot as your first choice, and fail to get it, the priority
of your second pick of say, Thursday
will come after that of another new team that chooses Thursday as their first

Why’s of OCUA’s 2002 Registration Form
1) Captain’s
& 2nd Contact Information
You will note that most of this
information is mandatory. Notification (offers of league vacancies, problems,
etc.) will be made using this information, primarily through email as well as
Ultimate Happenings and the OCUA website.
Please keep OCUA informed of any
changes to this information!
Wherever possible, try to get last
year’s captain to fill out the form. It will help avoid any confusion on OCUA’s
part. If this is not possible, please keep the information as close as possible
to last year. Contact information/Team Names can always be changed at a later
date. As returning teams have different privileges than new teams, please
ensure that we can tell the difference between a returning team with a new name
and a new captain and a brand new team to OCUA summer league.
2) 2002 Choices
You will have the chance to make
three selections for possible nights and ratios of play. Your first choice will
be given the greatest weight. You will be contacted before the Captains Meeting
and told if you have gotten a spot. If you have then you must attend the
Captains meeting at Brookfield HS at 7pm, were the remainer of your team fees
will be collected, and you will have an opportunity for questinos and answers
with you co-ordinator. NOTE: If you
are given given your first choice and do not take it your deposit is forfeit.
If you are given your second choice and do not take it then 50% of your deposit
is forfeit, if you are offered your third choice and decline it then yuo will
be subject to a $25 processing fee only.
EACH TEAM REQUIRES A SEPARATE FORM AND DEPOSIT.
For each team you wish to submit for
consideration for a spot in the OCUA summer league you must fill in separate forms and submit separate deposit
