What is an Incident?

Any outcome/experience/issue or complaint from a participant, parent, guardian, or spectator: For instance: Example 1: A player falls and lands in an odd position with no contact from another player on the field, they are unsure if they have a sprain/strain/fracture.

• Form required: Injury/Accident Report Form.

Example 2: A member required on-field first aid from a previous injury with no contact from another individual

• Form required: None, as the injury/accident did not occur during an OCUA sanctioned event/league.

Example 3: Two members participants run into each other on the field and one of them hits their head (with no other injuries)

• Form required: Suspected Concession Report Form.

Example 4: Two members collide, one or both leave the field due to injury sustained during the collision and requires medical attention. An injury (sprain/strain/fracture) is assessed and the player will be out of play for at least 2 games

• Form required: Injury/Accident Report Form.

Example 5: A member needs a band aid from a cut that occurred the previous day.

• Form required: None, as the injury/accident did not occur during an OCUA sanctioned event/league.
