2012 OCUA AGM - Proxy Form

Please find attached the proxy documents for the 2012 AGM. The 2011 AGM Minutes and the 2012 Agenda will be posted by next Monday, September 17, 2012. Please note that if you intend to vote in person at the AGM (Sept 25, 7:00pm at 1755 Merivale Road), we will have copies of the ballot ready for you - no need to print extras. If you do plan on attending, please get some team mates to fill out proxies!
